Other ways
Two more things worth knowing.
Ask Stars, on every page.
Ask Stars is the button in the header on phones and the round badge in the corner on larger screens. It points you to the right place on this site and to a real person at Stars. It does not diagnose or decide eligibility.
Why there is no contact form
This website runs no advertising trackers and no third-party forms. The Early Start request is the one form, built for the one thing most families and clinics need. For everything else, a call or an email reaches a person.
For a medical emergency, call 911. For urgent medical questions, contact your child's doctor.
Assessment requests
For an assessment request, start here.
The online Early Start request is the form. It takes a few minutes, works on a phone, and is available in English and Spanish. Families can begin directly. Professionals can refer.
